09:05 to 09:25 Sinus Elevation: Lateral Window vs Versah Crestal Approach. Is the Classic Technique Becoming Outdated?

We will discuss the Versah Crestal Approach for Sinus Elevation and how it has, on many occasions, replaced the classic Lateral Window Sinus Elevation Approach. Several clinical examples will be presented analysing the Versah Crestal approach to demonstrate how and when it can be used to approach sinuses with minimal vertical bone. We will also discuss the current indications of the Lateral Windows Sinus Elevation approach in my practice and if it is still relevant for specific cases.

09:25 to 09:45 Osteoplasmology - Natural Bone Growth in the Maxillary Sinus

I have been developing a novel technique for osseous augmentation in the maxillary sinus. This approach uses a specifically designed nozzle to deliver a PRP gel formulation, into the sinus, using an internal lift approach thus negating the need for a lateral window. This patient-centred approach increases acceptance of treatment, greatly reduces surgery time, reduces post-operative symptoms and recovery time, and allows for a completely autologous procedure. Early results show an increase of 5+ mm in the sinus floor bone thickness. Combined with the use of short implants it opens up a host of treatment opportunities that were, perhaps, beyond the remit of treatment in general practice. Results are preliminary but there is potential for an interesting new technique.

Callum Graham

Dr Graham qualified from Newcastle University in 1999, having changed careers from civil engineering. He has been placing and restoring implants since 2004 and has developed a keen interest in augmentation procedures without the use of animal products. His area of interest is in autologous maxillary sinus augmentation without the need for a lateral window approach.

Currently, he is working as an associate near Glasgow, having sold his practices to move away from practice management and concentrate on clinical dentistry, incorporating lasers and CEREC into his daily working practice.

09:45 to 10:05 A Novel 2 Part Bridge: Countering Trismus and Extensive Alveolar Resection after Curative Squamous Cell Carcinoma Surgery

Introduction
A 73 year old female patient underwent mandibular resection with placement of 3 dental implants prior to radiotherapy, as treatment for Squamous Cell Carcinoma of the left anterior mandible.

Methods
Surgical placement of 3 dental implants post mandibular rim resection. The apical aspects of the implants were congruent with the inferior border of the mandible. An extensive resection required the prosthetic abutment to be very deep and fitted with a very long screw to meet the implant fixture. Trismus meant no implant driver would fit in the mouth.

Results
The patient was rehabilitated with a novel and innovative custom 3D digital designed implant-supported 2 part bridge.

Conclusion
Trismus in combination with extensive resection presents challenges to placing implant supported prostheses. Creative digital planning and ingenuity of bridge design, in conjunction with dental lab technicians, was crucial to managing this complex case.

11:15 to 11:35 Non-Surgical Management of Peri-implant Mucositis; A Team Effort

The most common implant complications are biological in origin, with 65% of all peri-implant diseases associated with bacterial colonization of implant surfaces (Roos-Jansaker et al 2003, Renvert & Polyzois 2015). There is strong evidence from animal and human experimental studies that oral biofilm is an important etiological factor for development of peri-implant mucositis (Pontoriero et al 1994, Zitzmann et al 2001, Salvi et al 2012, Renvert et al 2018, Araujo & Lindhe 2018) and if treated non-surgically, may be completely reversible (Heitz-Mayfield & Salvi, 2018). Peri-implant mucositis, if left untreated can progress to peri-implantitis with less predictable clinical outcomes and increased risk of implant failure.

This presentation will focus on timely and effective interventions as the first line of defence of peri-implant mucositis, with a view to prevent (Jepsen et al, 2015) or delay progression to peri-implantitis (Lang & Berglundh, 2011).

11:55 to 12:15 Worst Nightmare in Implant Dentistry

An anterior single central incisor, an external hex implant placed fourteen years previously, a digital impression and restoration….. surely this should be a predictable treatment. A clinician, a technician and the patient give their account of this extremely challenging case in order for you to learn from the errors made. A case of misunderstanding the complexity at the outset, how communication skills let us down and how we would improve things if we were to have a similar case in the future.

Steve Campbell

Steve is a GDC registered dental technician with over 27 years dental experience, and a passion for the life changing service the dental team provide to our patients. Working with skilled teams and some of the most highly regarded dental surgeons in the UK, he provides solutions for all aspects of restorative dentistry, especially implants and the new digital and CAD/CAM workflows. Over time his passion for dental technology has evolved and intertwined with his love of cutting edge technology to the point where he found it possible to open and run a fully digital dental laboratory with his team at Nexus Dental in 2015. Since then the opportunities and challenges keep him very busy! When not in the lab he will usually be found at a dental event either learning from his peers or representing one of the many dental organisations he is fortunate enough to have been invited to work with, including his current role as President of the Dental Laboratories Association.

14:25 to 14:45 Modern Approaches to Manage the Emergence Profile and Generate the Soft Tissues Around Implants

Dental implants represent a unique complex system of human hard and soft tissues, mechanical components, and oral bacteria/biofilm. Therefore, many different factors are critical for successful outcomes when placing, restoring, and maintaining implants.

Prosthetic-driven implant placement is a well-known essential prerequisite while a growing body of evidence points towards the importance of the structure, anatomy, and dimensions of the peri-implant soft tissues and the emergence profile of the restorations for the long-term implant health and stability.

The aim of this lecture is to discuss the concept of developing a customised emergence profile as a standard of care for every single individual implant patient and to present up-to-date methods and techniques that can assist clinicians not only to place implants in the proper restorative position but also to simultaneously design, generate and condition chairside the peri-implant soft-tissues in simplified, predictable, reproducible and effective ways.

14:45 to 15:05 The Dynamic Screw – More Than Just a Get Out of Jail?

As screw retention has become the standard in implant dentistry, dynamic screw options are becoming increasingly available as both original components and through third-party manufacturers.

The Dynamic screw is often thought of as the solution to screw retention for implants in a less than ideal position. In viewing them in this way, are we missing an obvious solution to the problem of positioning the immediate implant in the aesthetic zone for ideal primary stability and screw retention at the same time? While some implant manufacturers have addressed this problem in the design of their implant, many have not. The solution presented here is potentially applicable to a wide range of implant systems.

Cases are presented in which the use of dynamic screw restorations were factored into the positioning of immediate bone level implants. The potential advantages of this approach are discussed along with a system of temporisation.

15:05 to 15:25 Dental Implants in Irradiated Bone

Patients who have had treatment with radiation in the head and neck can have long term implications to the bone that remains, such as increased risks of osteoradionecrosis.

Once thought to be a complete contraindication, dental implants can be placed into bone which has been irradiated with good survival and be of great benefit to these patients to support or fix various prosthetics.

With oral cancer rate still being high in the UK, these group of patients may become more common in primary dental care practices.

This presentation is a case study outlining the challenges I faced during planning, a summary of my research including published information on expected survival rates, proposed general guidelines for surgical treatment phase for assessment of suitability, and long-term maintenance.

Russell Colclough

Russell completed his undergraduate degree at Newcastle dental school in 2003.  After completing his vocational training under the care of the inspirational Ken Harris, he continued general practice briefly before pursuing some longed-for experience in maxillofacial surgery.  Since then, he has worked in private dentistry with a particular interest in the restorative and surgical aspects of dental implantology.  Since finishing the curriculum at the famed Kois centre in Seattle, USA, Russell continues to embrace their philosophy in the care he provides to his patients.  This approach lead him to winning in the best young dentist category and continues with the same passion now aiming to be one of the best middle-aged dentists.

16:30 to 16:50 Bone Augmentation: Success or Not a Success? That is the Question

Treating a severely resorbed edentulous space with GBR and soft tissue augmentation remains one of the most challenging and demanding techniques. This is due to the high expectation of the patient and clinician, treatment planning, patient selection, lack of understanding of anatomy, and potential complications.

Good knowledge of surgical technique, bio-materials, type of defect, post-operative management, and managing complications are vital to success.

It is also important to understand the role of healthy connective tissue, which plays a big role in the outcome and also the long-term success.

In everyday implant practice, it is common to see patients with mild to severe horizontal and vertical bone and soft tissue defects and single to multiple edentulous spaces.

I will discuss cases not only with a successful outcome but also with the not so successful - with horizontal and vertical hard and soft tissue defects, treatments and complications, and appropriate management.

The focus of this lecture will be on treatment planning, diagnosis, surgical techniques, and potential complications.

16:50 to 17:10 Anatomically Guided Strategies for Reconstruction of the Severely Atrophic Maxilla

The atrophic maxilla can pose challenges for the experienced implant clinician due to the insufficient availability of alveolar bone that often excludes successful provision of conventional implant treatment. Conceptually there are two possibilities of rehabilitation; staged grafting or graftless solutions. This presentation will discuss the anatomically guided, evidence-based, strategies for reconstruction of the atrophic maxilla including maxillary sinus grafting, onlay grafting, short and tilted implants, pterygoid, trans-sinus & zygomatic implants and 3D printed subperiosteal implants. Indications and success of maxillary sinus grafting will be compared with that of the modern ZAGA zygomatic implant technique to help the clinician use an evidence-based decision-making process when treating this challenging group of patients.
