09:05 to 09:25 Use of a Novel Implant design in Synergy with an Alloplastic Bone Regeneration Material in the Anterior Maxilla and Mandible

Three case studies providing a detailed overview of the use of a novel dental implant combined with alloplast GBR, immediate placement, and loading in the aesthetic zone.

All three cases will present unique elements within the use of these protocols. For example, the first lower anterior tooth, the use after case work up apicectomies and cyst enucleation/GBR, and the use for a single central incisor.

Dominic O'Hooley

Dominic O'Hooley qualified as a dentist in 1993 and has been placing and restoring implants since 2008.

His peripatetic practice is limited to implants and he is particularly interested in immediate loading within the aesthetic zone, complex bone grafting cases, and sinus work.

He mentors nationally and has presented both nationally and internationally on a wide range of implant-related topics.

Dominic is married to Rebecca with five children.

09:25 to 09:45 A Novel Approach to Immediate Molar placements

Immediate molar placement was a myth over ten years ago - traditionally a delayed approach was taken with extraction followed by a plethora of socket preservation techniques which inevitably resulted in some degree of resorption which in turn led to requirements of sinus grafting or lifting.

My presentation will explain the following:

1) How implant design can allow for immediate molar placement with insertion through the optimal prosthetic position given the thread designs available today.

2) A step-by-step surgical protocol to allow delegates to place immediate molar implants in the inter-septal bone and then stabilise this with precision within the socket.

3) We will discuss various augmentation techniques and soft tissue colour and speak about the way we can command fibroblastic activity to close around our implants.

Avik Jonathan Dandapat

Dr Avik Dandapat completed his BDS from Birmingham University in 2001. After a short period in general practice, he embarked and attained a  membership of the general dental practice at the FGDP and completed a diploma in implant dentistry as well as a advanced certification in 2007. This was then followed by a Masters in Implant dentistry from the University of Leeds.

Avik has been at an exclusively implant dentistry-based practice for the past 15 years and runs the Institute of Implant Dentistry in Berkshire.

In total, Avik has placed and restored 4000 implants and his MSc thesis was focused on immediate replacement of teeth. Most of his work is immediate placement and advanced full arch rehabilitation cases.

09:45 to 10:05 Immediate Placement With/Without Loading in Molars: Does it Have a Place in Our Day-to-Day Practice?

Immediate placement with/without loading in anterior teeth including premolars has become a common practice. Momen et al in 2010 analysed that immediate placement with the loading of a single mandibular molar implant had no significant difference in it’s success when compared to the delayed approach. However, we don’t see this approach being used widely due to anatomical considerations like maxillary sinus, ID canal, root morphology, etc. Is there a place for immediate placement with loading in maxillary and mandibular molars with or without loading? What are the factors we need to consider? What protocol can we use? We need more consensus on these dilemmas and the ADI Members National Forum is a great platform to start this debate.

10:05 to 10:25 Immediate Implant Dentistry: Fad or Fiction in 2021

This lecture aims to examine the concepts of immediate implant dentistry especially in the aesthetic zone and whether after more than two decades of this technique we are any closer to maintaining bone loss and soft tissue and having longer-term implant success.

Aims:

  • To examine the current immediate implant placement techniques (Hard and soft tissue)
  • To comment on the success of immediate loading concepts
  • Is zero bone-loss concept: miracle or myth
  • Where do we go from here?

Objectives:

  • To understand the aspects of immediate implant dentistry that are evidenced-based
  • To examine the importance of adjunctive supplementary procedures related to immediate implant dentistry that are fundamental to its long term success

11:15 to 11:35 "Sticky Bone" Using Platelet Rich Fibrin (PRF)

Platelet Rich Plasma has been used for many years in orthopaedic surgery, injected into osteoarthritic or traumatised joints to help patients accelerate their recovery.  A slight modification in the preparation of the patients’ blood has produced ‘Platelet Rich Fibrin’ (PRF), which has been used in dentistry for over twenty years.

This presentation will examine the advantages of complementing implant surgery with PRF.  From extraction sockets to immediate implants, bone grafting to sinus lift treatments, PRF has a part to play in aiding the healing and, in my opinion, making the actual surgery easier.

The talk will focus on using PRF to make “sticky bone”, demonstrating the advantages in handling and healing with a case presentation.

11:35 to 11:55 Bone Substitutes - Are They Really?

As clinicians, we often use bone augmentation materials without considering what they are, how they work, and what we really want them to do for us? In a presentation combining clinical slides and animations, Nigel Jones will try to help us think about which of the many materials available to us will be most effective in achieving treatment aims, and whether they actually do what it says on the tin (or bottle).

Nigel Jones

Nigel Jones operates a referral practice in Abergavenny limited to the assessment, placement, and restoration of dental implants. He is also the cohort director for the FGDP Implant Diploma in both London and Hong Kong which extends his contact with friends and colleagues from the ADI. 14:05 to 14:25 A Critical Evaluation of Deviation from Planned Position to Actual Position of Implant Site Preparation

Digital technology has transformed implant treatment planning and has enhanced the accuracy of implant placement. Static guides provide a fixed pathway for site preparation. In contrast, dynamic navigation permits the surgeon to alter the position of the osteotomy away from the computer-generated planned position and to still monitor the site preparation in real-time in 3D.

This retrospective study reflects on implant site preparations by one operator since 2018 using dynamic navigation. Through studying recordings, it was possible to assess the endpoint proximity and angulation of osteotomy compared to the pre-treatment planned position. Data in the maxilla and the mandible, differing sites, healed and immediate cases, and where bone augmentation was adopted, had been examined.

The results demonstrate differing degrees of variation from the final drill position to the planned position. The explanation for elected and non-elected changes to the computer-generated treatment plan are discussed.

David Burgess

David BurgessHaving graduated from Manchester Dental School in 1985, David started placing dental implants in the year 2000. He was an early adopter of digital technology being the first dentist in the UK to purchase a 3D intraoral scanner, and in 2015 was the first in the UK to place implants using dynamic navigation. Since then, David has placed over 3500 implants using Navident, and as a Master Clinical Trainer has run many courses in dynamic navigation with delegates attending from Europe, the Middle East, and Asia. Having been the first surgeon in the world to carry out many different implants and endodontic procedures using dynamic navigation, David has had numerous case studies published globally. More recently, he has carried out reflective studies into behaviour patterns during implant site preparation and will present a discussion based on these findings at the 2021 ADI Members Forum.

14:25 to 14:45 Getting the Best from your CBCT Examination

How do you maximise the value of your CBCT examination with some quick and inexpensive techniques? When do you need CBCT and when can you use conventional radiographs? What are the alternatives for imaging prior to dental implant placement and what do current guidelines say? What are the anatomical variations to look out for on our CBCT images? Andrew will address these questions and present some practice-friendly hints and tips for using CBCT effectively.

14:45 to 15:05 Evaluation of Tooth Supported 3D Guided Dental Implant Surgery: Impact on Surgical Precision and Collateral Damage to the Inferior Alveolar Nerve

The aim of this study was to compare the effect of freehand (FH) and fully guided (FG) surgery on the accuracy of implants placed in close proximity to vital structures such as the inferior alveolar nerve (IAN) using R2 Gate ® system. Cone beam computed tomography (CBCT) and intraoral scans of six patients who have had previous dental implants in the posterior mandible were used in this study, to produce replica models. Implants were placed using FG and FH methods on the respective models and the results were compared with 3D planned positions. The angular deviations were calculated and the distance to the IAN was measured. Whilst surgically acceptable overall accuracy was obtained with FH, better margin of error was observed with FG surgery with respect to the angular deviation and controlling the distance of the implant to the IAN using R2 Gate ® system.

15:05 to 15:25 Static Shock - Are We Being Guided to Failure?

Guided implant surgery is widely seen as the future of implant dentistry and is becoming increasingly accessible to practitioners of all levels of experience. The accuracy of digitally planning and surgery using static surgical guides has been proven time and time again in the literature even at this early stage in the adoption cycle.

Guided surgery is not without its drawbacks however, excess heat generation leading to bone necrosis, the misuse of the guides, or a myriad of intra-operative vicissitudes can lead to catastrophe.

I will present the findings of my systematic review, published in 2020 in the British Journal of Oral and Maxillofacial surgery to answer the question, do implants that are placed using a digitally-designed static surgical guide offer comparable survival rates to those placed by all methods?
