Registration

Alternatively, please print the PDF application form, complete it and post/fax to: 020 8487 5566 or post a cheque made payable to the "ADI" to: 98 South Worple Way, London, SW14 8ND.

Cancellation

Cancellations will be accepted in writing until Friday 29 October 2010 and are subject to an administrative charge of £50. After this date no refunds can be guaranteed.

Accreditation

4 hours of verifiable CPD. Certificates of Attendance will be available to download after the event.

Please confirm your details in order to complete the evaluation form and retrieve your CPD certificate. The ADI will treat your responses confidentially - no one outside of the ADI office staff will have access to your name.

In case of difficulty, please ensure you have entered your name exactly as given on your ADI receipt.
