Why We May Not Need To Worry About Peri-implantitis Quite As Much As We Are Being Led To Believe

Over the last five to ten years there has been considerable discussion about bone loss around dental implants, peri-implantitis and peri-mucositis. Various groups, including the ADI, have had consensus conferences and produced statements and guidelines on the management and maintenance of implants. Despite this there still remains significant confusion on the subject. There is a section of the dental implant community who don't agree with most of the proposed guidelines to date and who have a very real concern that if following current monitoring "guidelines", many patients will be receiving treatment that they don't need.
In the 2016 Periodontology 2000 publication, a paper was presented by part of the team at Edinburgh Dental Specialists that made a convincing argument against the use of bleeding on probing and pocket depths as effective monitoring indices.
This presentation will be looking at the current "zero tolerance" for plaque, BOP, pocket depth over 4mm and pus exudate - asking where is the evidence? Does it really hold up? How should we be assessing our implants? And, if bone loss has been seen to occur, how should we manage it?
In addition we will also look at “risk factors” and specifically where periodontal diseases factor into the development of bone loss around implants.
Aims and Objectives:
- To understand the reasons for bone loss around implants
- To understand why the current "guidelines" exist and the "evidence" to back them up
- To understand why the "evidence" may not be being interpreted appropriately
- Delegates should have a greater understanding of the reasons for bone loss around implants and why the vast majority is not due to peri-implantitis.
- Delegates will understand why there is confusion and why current guidelines may be misleading
- Delegates will be made aware of a proposed simple monitoring system for all implants
- Ideas on management of bone loss around implants will be presented
Biography
Dr. Kevin A. Lochhead
B.D.S.(LOND),D.G.D.P.(UK)
Dr. Kevin Lochhead is a Specialist Prosthodontist who has spent over 25 years working in private practice.
Kevin qualified from Kings College London in 1987. In 1993 after significant private training in advanced restorative dentistry, both in the UK and North America, he established a city centre private practice, concentrating on aesthetics and advanced restorative care. In 2001 he was recognised by the GDC as a Specialist in Prosthodontics . The practice has since been developed into a multidisciplinary specialist “referral only” centre of which he is clinical director.
The growth and development of the practice was fuelled by Kevin’s passion and excitement for providing restorative care of the highest standard. He believes in the “team approach” to managing patients where specialist clinicians work closely together on each case. This way of working means that each clinician can concentrate on their specialist area, coming together to share their knowledge and plan cases accordingly. Patients care is not limited to the expertise of any one clinician and particularly challenging cases can be easily managed.
“Edinburgh Dental Specialists” is an eight surgery practice over 5 floors, comprising 12 specialists covering all restorative and surgical specialities. Integral to the development of the practice has been establishing an onsite dental laboratory with a team of technologists who produce all the restorative work for the practice.
This presentation is the collective interpretation of current literature and evidence by the team of specialist periodontists, prosthodontists and surgeons at Edinburgh Dental Specialists.
