Soft tissue deficiencies around implants impact aesthetics and often lead to escalating problems. Early recognition and intervention with minimally invasive soft tissue surgery not only enhances aesthetics, but also reduces the risk of undesirable sequellae associated with conventional surgical approaches around implants with more advanced problems. The application of a minimally invasive soft tissue grafting method will be shown for treatment of soft tissue problems around implants and the indications for use of this technique will be outlined.

Dr Allen has served as President of the American Academy of Esthetic Dentistry, the American Academy of Restorative Dentistry and the American Academy of Periodontology Foundation. He is the recipient of the Master Clinician Award from the American Academy of Periodontology and the President’s Award for Excellence in Dental Education from the American Academy of Esthetic Dentistry. Currently, he is the Periodontal Section Editor for the Journal of Esthetic and Restorative Dentistry and serves on the Editorial Boards of the Journal of Periodontology and the International Journal of Periodontics and Restorative Dentistry. Dr Allen is founder of the Center for Advanced Dental Education in Dallas, an educational facility where he teaches surgical technique courses. He has over 100 publications and has presented numerous lectures and surgical demonstrations worldwide.

Osseointegrated oral implants have been successfully reported for over 40 years. The prime dictate prerequisite to predict long-term success is a sufficient volume of healthy bone at recipient sites. However, sufficient bone volume is frequently lacking as a result of trauma or infectious diseases such as advanced periodontitis. Regardless of the numerous surgical techniques described in the literature that aim at regenerating bone in a vertical and horizontal dimension, clinicians still struggle when faced with challenging vertical defects.
Vertical bone loss in partially edentulous patients constitutes a major challenge due to anatomical limitations and technical difficulties. Extra or intra-oral harvesting of autogenous bone constitutes a high degree of patient discomfort and morbidity, as well as a limited supply.
Advances in tissue engineering may offer alternative solutions to resolve bone and soft tissue defects. The principal aim in hard tissue regeneration would be to eliminate the need for autogenous bone harvesting (both intra and extra-oral) and possibly eliminate the non-resorbable membrane.
The presentation will cover the history behind the rationale of applying the tissue engineering concepts to the everyday clinical work on patients. Pre-clinical studies will be shown who have been the backbone of the clinical work using these novel techniques.

Regenerative and implant therapies allow clinicians to offer previously undreamt of treatment outcomes to their patients. However, the definitions of success following these treatments remain ill defined. Such success must be grounded in logic and clinical realities, and offer high degrees of long term predictability. This presentation will focus upon both the definitions of success in regenerative, implant and reconstructive therapies, and the diagnostic and technical prerequisites necessary for attaining such goals. Differential diagnosis, and clinically based hierarchies of material and treatment selection will be discussed in detail. The results of thousands of clinical cases will be discussed and critiqued.
Dr Fugazzotto received his DDS from New York University in 1979 and a certificate in advanced graduate studies in periodontology from Boston University in 1981. Since that time Dr Fugazzotto has maintained a private practice in periodontics and implant therapy in Milton, Massachusetts. Dr. Fugazzotto has authored or co-authored over 80 articles in refereed scientific journals, as well as authoring a monograph entitled “Guided Tissue Regeneration: Maximizing Clinical Results” and three textbooks entitled “Preparation of the Periodontium for Restorative Dentistry”; “Decision Making in Regenerative and Implant Therapies”; and “Periodontal Restorative Interrelationships: Maximizing Treatment Outcomes”. Dr Fugazzotto is an active member of many organizations, and is a fellow of the International Team of Implantology. Dr Fugazzotto is Senior Editor of Implant Realities and the US ITI Study Club Coordinator.
Dr Fugazzotto lectures nationally and internationally on a multitude of topics.

The Audience Response Session is an innovative addition to the ADI Congress. Chaired by Dr Michael Norton, a common treatment-planning dilemma will be presented to the audience, who will then vote on their preferred treatment option. Three speakers from the Congress faculty will then present each treatment option, using a combination of experience and scientific evidence to argue their case. Following an interactive discussion between the speakers and the audience, another vote will be taken, to establish whether the session had changed the views of the delegates. Dr Norton will conclude the session, by summarizing the arguments and delivering his views based again on documented data.

His considerable portfolio of research has been ground-breaking and he has become one of the most sought after lecturers in his field. Since 1989 Michael has dedicated all his clinical and postgraduate time to the practice and study of implant reconstructive dentistry.
Michael is Secretary, Board Member and Fellow of the Academy of Osseointegration (AO) and is Past President (1999-2001) and Honorary Life Member of the Association of Dental Implantology (ADI), UK. He is past editor of the AO’s Academy News and is currently Associate Editor of the International Journal of Oral & Maxillofacial Implants (JOMI). Michael is widely published in the literature including one of the earliest Quintessence textbooks on the subject published in 1995. The placement of a single implant is often used to demonstrate how it is possible to obtain a high degree of esthetics with dental implants. However the invaluable contribution of the adjacent natural teeth can never be ignored or underestimated. Supra-crestal fibres attached to the root cementum of juxtaposing teeth are entirely responsible for the maintenance of the tip of the interdental papillae which frames our implant-supported crown and helps to ensure an optimal esthetic outcome.
Placement of adjacent implants is much more problematic since it is widely recognised that in the absence of root cementum and supra-crestal fibres it is not possible to maintain a full length papilla and thereby not possible to frame our implant-supported crowns with an optimal gingival profile.
This presentation will consider how surgical technique, planning and timing of the placement of adjacent implants, as well as choice of component hardware can all contribute to achieving an optimal outcome with regard to both pink as well as white esthetics in the adjacent implant scenario.
