Ricardo KernDr. Kern graduated from the Federal University of Paraná (UFPR), one of the most respected universities in Brazil. He obtained a postgraduate degree in Periodontology in 2003 and a Master’s degree in Oral Rehabilitation from the State University of Ponta Grossa in 2007.

Firmly entrenched in the high aesthetic demands of Brazilian dentistry, Dr. Ricardo Kern focuses his work on the demanding disciplines of soft design and reconstruction of fabrics.

He has extensive experience in immediate placement of timed implants, from single tooth cases to full arch rehabilitations. He has presented didactic programs and hands-on workshops on soft tissue design, preservation and reconstruction at various seminars and conferences.

16:00 Periodontal Re-generation: Teeth Vs Implants

Saving teeth versus removing and replacing with implants is a clinical decision made depending on the circumstance. Though every circumstance is different, it comes down to a single, simple question: what is the most rational solution for the given issue and the patient? It is essential that we all recall that the main purpose of our profession should be treating pathologies and saving teeth whenever possible, not to extract teeth and replace with implants. When we extract and replace a ‘maintainable’ tooth, the patient pays a greater biologic and economic cost than saving it. A tooth should be replaced only when irrational to treat. Modern regenerative surgery has been demonstrated to be effective in improving the prognosis of teeth with severe loss periodontal support. The edge of periodontal regeneration is “to give hope to hopeless teeth”. Teeth that would have been declared hopeless some years ago today can be treated and saved because of the extraordinary development of powerful regenerative therapy. Hopeless teeth treated with regeneration have high chances of long-term survival within a stringent supportive periodontal care program.

Learning Objectives

  • Rationale for applying periodontal regeneration to a “hopeless” tooth
  • A Stepwise approach to successful regenerative treatment
  • Patient benefits to saving a tooth versus extraction and replacement

09:05 The Management of Complicated Defects in the Aesthetic Zone

The aesthetic zone presents several very specific problems which are exacerbated following tooth loss. The loss of bone and soft tissue, as well as the natural gingival contour, makes replacing the teeth with implants a huge challenge. The attention to detail regarding new bone growth followed by soft tissue grafting and prosthetic soft tissue development are key to achieving an excellent aesthetic result that is stable over the long term. There are a number of key parameters that need to be followed to achieve the predictability of our results. The decision-making tree is paramount and differs from patient to patient, and all of these factors will be brought together in an algorithm that allows one to choose the best treatment option for the patient. From Digital Smile design to choice of bone graft material to timing of soft tissue surgery. The lecture will share more than 25 years of failure and success, but with a constant striving for improvement and excellence.

09:45 Clinical Reality Of Vertical Ridge Augmentation

The three-dimensional reconstruction of hard and soft tissues has become a real challenge in the daily practice of modern oral implantology. Diseases such as periodontitis, peri-implantitis, pathologies and even major trauma, generate vertical defects that today can be reconstructed aesthetically and it is an opportunity for the patient to access these treatments. For this reason, it becomes essential to know how to diagnose, manage the technique and even manage possible complications, since it is well known that it is a very sensitive technique and depends a lot on the operator.

Learning Objectives

  • Diagnose and learn to know when and how these vertical defects can be treated
  • Know the technique and step by step that will give us more predictable results
  • Learn to avoid complications, and if they occur, know how to manage them to avoid total graft loss
  • Maintain vertical augmentations over time, with good peri-implant health.

11:00 Zero Bone Loss Concepts: Immediate and Delayed

Mucosal tissue thickness was shown to be the factor having impact on crestal bone stability. Even platform switching of the implant-abutment connection does not reduce crestal bone loss, if soft tissues at the implant placement are thin.

It is suggested that thin tissues might be thickened during implant placement, thus reducing bone resorption. If bone height is not sufficient, vertical augmentation of the soft tissue is recommended with different materials - autograft, a xenograft or a dermis-derived allograft.

Further, it is important to preserve bone levels after prosthetic treatment. Recent research has proved that the deeper the position of the margin, the greater amount of residual cement is left undetected. The relation between position of cement excess in the peri-implant sulcus, periodontal status of the patient and severity of peri-implant disease is explained. To avoid cement excess, finished implant restoration with occlusal opening is cemented on titanium base in laboratory and restoration is attached to the implant by an abutment screw. Zirconium as a material is considered the best for peri-implant soft tissues. However, it’s evident that it must be treated in special manner and polished.

Learning Objectives

  • To understand the potential advantages and disadvantages of surgical and prosthetic techniques in the light of recent data from the literature regarding different strategic option procedures in full-arch rehabilitation
  • To understand the diagnosis, treatment planning, patient selection criteria and surgical and prosthetic management of each type of surgical and prosthetic technique
  • To understand the critical elements involving the decision between FP1 and FP3 prosthesis design, giving emphasis to clinical cases of both options
  • To know how to simplify the decision-making process by understanding the importance of treatment plan the Carames Classification, a patient-centered aid-decision tool for a fixed and removable full-arch implant rehabilitation of the maxilla and the mandible, will be presented and discussed in different clinical scenarios.

13:20 The Immune System Behind Osseointegration and Marginal Bone Loss

Osseointegration is an immune controlled bone shield off reaction to an implant to protect nearby tissues. Threats to osseointegration may occur due to septic and aseptic reasons. Patient factors such as genetic reactions, smoking or the consumption of certain medications may combine with clinical reactions to surgical and prosthodontic trauma and implant reactions in form of corrosion or fracture that taken together may lead to bone resorption. If the summed trauma due to these factors will be too large, the immune system may surrender and shift over from shield off to implant rejection. In the latter stage, immune defense against bacteria goes down and the implant will now be threatened by aseptic as well as septic attacks. In this manner, the immune system is behind osseointegration as well as threatening marginal bone loss around the implant.

Learning Objectives

  • Osseointegration is an example of an immune response to an individual implant

  • When in balance, the immune system will protect against septic reactions

  • The immune system may shift over to implant rejection due to the sum of traumatic factors

  • If implant rejection by the immune system, bacterial attacks will add to previous aseptic loosening of the implant

Tomas Albrektsson
14:00 Clean Implant

A quality assessment study conducted in collaboration with the Charité University Medicine, Berlin, revealed an alarming number of sterile packaged dental implants with factory-made contaminants among the 100+ tested samples. Significant amounts of hazardous impurities and even numerous nickel-, tungsten- or chromium-containing metal particles raise serious concerns among dentists. Unfortunately, the majority of dental practitioners only have little unbiased information about the surface quality of the implants they use in their daily practice. The costs for inferior quality implants will be borne by patients and clinicians. Inflammatory processes, bone loss and even implant failure as possible consequences of impurities and uncontrolled foreign body reactions affect the patients directly. Additionally, dentists risk losing their reputation or may even face lawsuits and claims for damages.

Learning Objectives

  • Learn more about the biologic response on foreign bodies and the relation to peri-implantitis
  • See why residue-free implant surfaces are crucial for a successful osseointegration
  • Get information about the extent of contaminated implants in the market and learn how to avoid using implants of inferior quality.

15:15 Classification and Surgical management of Implant Esthetic Complications

Esthetic complications of dental implants can have a major negative impact on patient’s qualify of life and perception of implant therapy.

A recent classification of peri-implant soft tissue dehiscences has been proposed with the goal of providing a uniform system for the diagnosis of these conditions, and, at the same time, guiding clinicians on the selection of the most indicated treatment approach for each type of esthetic complication.

This lecture will present the new classification of peri-implant soft tissue dehiscences/deficiencies and will discuss the rationale, treatment planning and soft tissue grafting techniques for the correction of implant esthetic failures.

Learning Objectives

  • Learning the recent classification of peri-implant soft tissue dehiscences

  • Understanding the role of soft tissue phenotype on peri-implant esthetic, tissue stability, and health

  • Understanding how to select the most appropriate treatment approach for each type of peri-implant soft tissue dehiscence

Mahrukh KhwajaDr. Mahrukh Khwaja is a dentist, positive psychologist, accredited mindfulness teacher, and the founder of Mind Ninja, a first-of-its-kind wellness start-up dedicated to improving mental health and resilience among dental professionals.

With over 12 years of clinical experience “at the chair-side,” Dr. Khwaja has an insider’s perspective on the unique stressors affecting dental professionals. She also has first-hand knowledge of the lack of industry-specific mental health tools and support — something she discovered during her own bout with burnout. With nothing of its kind currently available in the dental industry, she envisioned a system of support that, just like daily flossing and brushing, took a preventive maintenance approach to mental health. Drawing on the neuroscience of well-being, and positive psychology, she developed Mind Ninja as a solution that would help fellow dental professionals build mental resilience, foster connection and meaning, and thrive.

Mind Ninja offers a comprehensive range of training, coaching, and wellness products that go beyond reducing symptoms of stress to generating a solid foundation of positive well-being. These tools are designed to help dental professionals thrive in any environment, ensuring that their own bright smiles come from a place of genuine mental health. Dr. Khwaja has delivered Mind Ninja’s bespoke workshops and well-being programmes to bring transformative change to a diverse range of organisations, including NHS Oxford Health, King's College London, Acteon, British Dental Association, and the Royal College of Physicians and Surgeons of Glasgow. Mind Ninja received Global Health Pharma's 2022 Award for Excellence in Innovation and was a shortlisted finalist at the Private Dentistry Awards in 2021 and 2020.

Mind Ninja’s flagship wellness products include the Mind Flossing Toolkit. This multi-purpose deck of well-being cards features positive psychology interventions that inform and inspire dental professionals to build on their strengths and develop a growth mindset as well as practise mindfulness, self-compassion, and gratitude, both in the dental clinic and at home. Mahrukh is also the author of the book Resilience and Well-being for Dental Professionals published by Wiley-Blackwell.

Dr. Khwaja has been named in the Top 50 in Dentistry two years in a row (2022 and 2021) by FMC for her work on Mental Health in Dentistry. Connect with Dr. Khwaja at the www.mind-ninja.co.uk or on @mindninja.wellbeing on Instagram

09:00 The Digital Approach to Full Arch Implant Dentistry

Digital dentistry has become very common in implantology and we have now reached a point where many clinicians routinely use digital workflows for single and multiple implant restorations.

Digital dentistry has allowed better planning, treatment and delivery of this treatment modality and now, this is possible with a fully digital workflow from initial diagnosis to final delivery. But we are still at a point where the common perception is that intra oral scanners cannot be used for full arch dentistry as they are perceived to be to inaccurate for this use.

  • When will intra oral scanners be accurate enough to use for full arches?
  • What are the workflows that we need to use for full arch fully digital restorations?
  • What are the advantages of fully digital, partly digital and analogue full arch workflows?

Learning Objectives

  • To understand fully and partially digital workflows and their use in practice
  • To understand the strategies for digital full arch restorations
  • To understand the advantages and disadvantages of full arch full digital workflow
