Single tooth implants in the aesthetic zone are amongst the most challenging cases to execute well. Attention to detail is paramount for predictable, long-term success. Once upon a time an integrated implant was deemed a success and it was enough that an implant remained integrated. This is not the case nowadays with aesthetically aware patients not only wanting the implant to remain integrated but also for the soft and hard tissue architecture to remain as they did before the tooth was extracted. Many techniques are employed to maintain and mimic nature, such as GBR and soft tissue grafts. Once integrated, provisional restorations are then utilised to tissue 'train' soft tissues for an ideal emergence profile and a predictable aesthetic outcome. It is far easier, however, to support and maintain the soft and hard tissue architecture from the day of extraction whether the implant is placed immediately or not.
This presentation will outline some innovative techniques and outside the box thinking to help maintain the aesthetic zone architecture from day 1, highlighting the importance of pre- and peri-implant provisionalisation. Several cases will be shown, including partial extraction therapies and socket shield. It will ultimately demonstrate that immediate implants in the aesthetic zone can be a predictable technique when strict protocols and techniques are adhered to.
